What is Spravato?

Spravato (esketamine) is an FDA-approved nasal spray for adults with treatment-resistant depression — depression that hasn't responded to two or more standard antidepressants. Unlike off-label ketamine treatments, Spravato is prescribed and administered in a certified clinic, under medical supervision, alongside an oral antidepressant.

It's the first medication of its kind specifically approved for treatment-resistant depression. Until Healtop opened Spravato certification, patients in our area had to travel hours to access it. Now they don't.

What to expect.

Spravato treatment happens in two phases.

Induction (first 4 weeks): Two sessions per week, supervised on-site at our Pounding Mill clinic. Each session takes 2 to 2½ hours from check-in to discharge. You self-administer the nasal spray under our medical team's supervision, then rest in a quiet treatment room while we monitor blood pressure and how you're feeling. You won't drive home afterward — please plan for a ride.

Maintenance (after week 4): Sessions taper to weekly, then every other week, depending on how you're responding. Your provider will adjust the schedule with you.

Throughout treatment, you continue your oral antidepressant. Spravato is added to your existing depression care, not a replacement for it.

Common questions about Spravato.

Is Spravato the same as ketamine?
Not exactly. Spravato (esketamine) is one specific form of ketamine, FDA-approved for treatment-resistant depression in adults. It's the only ketamine-related medication approved by the FDA for depression. "Ketamine clinics" that offer IV infusions are using ketamine off-label — not FDA-approved for depression, often not covered by insurance, and administered in widely varying settings.
How long until I might feel a difference?
That's an individualized clinical question, and it's something to discuss with our provider during your initial evaluation. We're not able to predict outcomes — but we can walk you through what the research shows and what to expect from the treatment timeline.
What are the side effects?
The most commonly reported side effects include dissociation (a temporary feeling of being detached), drowsiness, dizziness, nausea, and a temporary rise in blood pressure. This is why every session is medically supervised — we monitor you for at least two hours after each dose. Your provider will review the full safety profile during your initial visit.
Can I drive home after a session?
No. You'll need a ride home after every Spravato session. Patients can't drive or operate machinery until the following morning, after a full night's sleep. We'll help you plan transportation before your first appointment.
Do I have to stop my current antidepressant?
No — Spravato is used together with an oral antidepressant. That's part of how it's approved. You continue your existing medication and add Spravato to your care plan.
