What to Bring

Your first visit checklist.

Bring these items so we can get you started right away

Required Must Have

Photo IDDriver's license, state ID, or passport. Must match the name on your insurance.
Insurance CardFront and back. We'll scan both sides. If you don't have insurance, we offer self-pay and sliding scale options.
Copay PaymentCash, check, or credit card accepted. We collect copays at check-in per our insurance agreements.
Current Medication ListAll prescription medications, over-the-counter drugs, vitamins, and supplements — including dosages.

Helpful to Have If Available

Medical RecordsRecent lab results, imaging, hospital discharge summaries, or records from a previous provider.
Allergy InformationKnown allergies to medications, foods, or environmental triggers.
Immunization RecordsEspecially Hepatitis B, Tetanus/TDaP, Pneumonia, Flu, and COVID-19 vaccination history.
Family Medical HistoryMajor conditions in parents or siblings — heart disease, diabetes, cancer, stroke, mental illness, or addiction.

    Name, contact info, emergency contact, pharmacy, and basic demographics.

    Medical & Dental History

    Current conditions, past surgeries, family history, habits, and health screenings. This helps Sidney build a complete picture of your health.

    Consent to Treatment

    Standard consent authorizing examinations, diagnostic tests, and care. You always have the right to refuse any procedure.

    Notice of Privacy Practices

    Acknowledgement that your health information is protected under HIPAA. We'll explain your rights and how we safeguard your records.

    Authorization to Release Health Information

    Allows us to request records from previous providers or coordinate care with specialists. You control exactly what gets shared.

    Missed Appointment Policy

    Our policy for cancellations and no-shows. We ask for 24 hours' notice if you need to reschedule.

Can't complete forms in advance? No problem. Arrive 15 minutes before your appointment and our front desk team will help you through everything.

Forms can also be faxed to (276) 345-4274 or dropped off at the office before your visit.

Check In

Our front desk will verify your ID, scan your insurance card, collect your copay, and confirm your contact information. If you haven't filled out your forms yet, we'll hand them to you here.

Vitals & Intake

A team member will take your vitals — blood pressure, heart rate, weight, temperature. You'll have a chance to share why you're here and what's on your mind.

Meet with Sidney

Sidney Sparks, FNP, conducts a thorough evaluation. For primary care, this is a standard new patient exam. For OBAT patients, this includes a medical assessment — and if appropriate, you may start medication the same day.

Plan for 45-60 minutes total

Your Care Plan

You'll leave with a clear plan — whether that's primary care follow-ups, lab orders, prescriptions, a recovery support plan, or a combination. We schedule your next appointment before you walk out.

We Verify Before You Arrive

We check your insurance coverage before your visit so you know exactly what to expect — no billing surprises.

Bring Both Sides of Your Card

We need to scan the front and back of your insurance card. Make sure the card you bring matches your current plan — not an expired one.

No Insurance? No Problem.

We offer self-pay rates and sliding scale options based on income. Cost should never stop you from getting care. Call us to discuss.

Your privacy is our priority.

Everything discussed in our clinic stays between you and your provider. Your records are protected under the strictest federal privacy laws. We will never share your information without your written consent — not with employers, not with law enforcement, not with anyone.
